# Bessemer, Bain survey finds healthcare AI ROI landing in back office, not clinical work

*Healthcare artificial intelligence investments are delivering returns twice as fast as executives expected, but the financial gains are concentrated in administrative functions rather than clinical care, according to a new report from [Bessemer Venture Partner](https://app.clinicalterminal.com/company/101389358)s and Bain & Company.*

The report, authored by Bessemer Partner [Sofia Guerra](https://app.clinicalterminal.com/person/107519462) and [Steve Kraus](https://app.clinicalterminal.com/person/107519463), among others, surveyed 226 executives across 65 use cases at health systems, payers, and biopharma companies. More than half of respondents, 54%, saw material returns within the first year, the survey found.

The highest returns were in provider revenue cycle management, which delivered a 4.0x ROI, according to the report. In that area, 67% of provider respondents are running semi- or fully autonomous agents. In contrast, clinical use cases returned a 2.9x ROI, with only 4% of solutions operating with the same level of autonomy, Bessemer and Bain reported.

The authors attributed the gap in clinical AI adoption not to the technology's capability but to issues of trust, liability, and reimbursement. The survey found that 77% of respondents override AI suggestions more than half the time, and 58% said the treating clinician holds primary accountability for decisions influenced by AI.

The report also details a shift in how health organizations acquire AI. Forty-two percent of buyers have completed or are running a vendor consolidation process. Fewer than half of internally developed AI solutions are still maintained and in use, with the share of development done internally falling by 16 percentage points as healthcare-specific AI vendors gained 12 points. “Buyers have concluded that this is not work they want to own,” Guerra and Kraus wrote.

The move from experimentation to production is also affecting staffing. Half of the organizations surveyed have either cut headcount as a direct result of AI or plan to do so within six months, the report found. The average reduction is between 8% and 13% of the affected labor for given use cases.

**Why it matters:** The link between automation and job cuts is most direct in the back office. For provider organizations making headcount reductions, 73% named revenue cycle and medical billing as the affected departments, according to one analysis of the survey. This aligns with the report's finding that provider RCM has the highest ROI (4.0x) and the highest rate of autonomy (67%). The trend away from internal builds toward vendor consolidation marks a strategic shift for health system leadership. The report notes that buyers are moving from evaluating capabilities to buying implementations, and 42% are actively consolidating vendors to simplify their technology stack and reduce costs. Despite the administrative focus of current ROI, executives believe AI is already transforming patient care; the survey noted that 71% of executives now say generative AI has already transformed clinical treatment decisions.
