Cityblock Health to acquire Homeward; General Catalyst leads $116M Series E
Cityblock Health is acquiring Homeward, according to an announcement from General Catalyst Group Management, LLC. General Catalyst, which has backed both companies, said it is leading a $116M Series E for Cityblock Health to support scaling proactive, affordable, and accessible care.
The combined entity will reportedly use AI to free up more time for patients and serve the millions of Americans covered by government health programs, from urban areas like Brooklyn to rural counties. The announcement congratulated Cityblock CEO Toyin Ajayi and Homeward's Jennifer Schneider, M.D., M.S., among others.
Financial terms of the acquisition were not disclosed in the announcement. The deal brings together two companies focused on reaching populations that are often hardest for the healthcare system to serve.
